Delayed rupture of occult splenic injury: a case report

چکیده
The spleen is the most commonly injured organ after blunt abdominal injury (40-55%). Most patients presented acutely with symptoms of haemodynamic instability or acute abdominal signs attributed to haemoperitoneum. Delayed splenic rupture is defined as late occurrence of symptoms and signs, in patients who experience no initial haemodynamic instability or clinical symptoms, 48 hours or more after injury. The importance of delayed splenic rupture lies in the relatively high mortality rates (5-15%) as compared with those with acute splenic rupture (1%).

Occult splenic injury: delayed presentation manifesting as jaundice.
Delayed splenic rupture, though uncommon, is a recognised clinical condition and is associated with a significant mortality rate. In the vast majority of cases the patient re-presents with abdominal symptoms and/or signs of acute intraperitoneal haemorrhage. A case is presented of delayed splenic rupture presenting with jaundice attributable to intra-abdominal clot lysis.

discussion: This case highlights the delay with which an iatrogenic splenic injury can present. The most important indicator in this case, which could relate to splenic injury, was a scar near the splenic region following lung biopsy. Clinical problems after splenic rupture have been classified into three groups characterized by the delay in presentation and type of symptoms. Group one: acute r...
متن کاملDelayed and occult splenic rupture: a diagnostic puzzle.
Splenic rupture is a common complaint encountered in emergency surgery. Trauma is the most common cause of splenic rupture, while non-traumatic or occult splenic rupture (OSR) is a rare condition. The differential diagnosis weighs on treatment that ranges between close monitoring, splenorrhaphy, splenic conservation and splenectomy. We report a case of an 63-year-old man presenting with acute a...
متن کاملTrivial trauma and delayed rupture of a normal spleen: a case report
INTRODUCTION Although a majority of splenic ruptures present acutely with a known mechanism of injury, a minority of patients present days to weeks following trauma with a delayed rupture. Also uncommon is the atraumatic rupture, the vast majority of which occur in patients with underlying splenic pathology. A handful of cases of apparently spontaneous rupture of a normal spleen are reported; h...
متن کاملA case report on late presentation of occult dural lesions
Background: Occult dural injuries are rare and can occur as a result of major or minor head injury. These injuries usually manifest with cerebrospinal fluid rhinorrhea alone, or with meningitis and cerebral abscess, sometimes many years after the original injury. Case presentation: We present a case of occult dural injury with endocranial complications which occurred in a 34 year old man, wi...
